Cornwall FA would like to know what 12 to 21 year olds like and dislike about playing football in Cornwall. 

We also want to hear the views of our players on continuing to play through to adult football in the county.  This feedback will help Cornwall FA to develop football in Cornwall and keep more young people playing and enjoying the game into their adult years. 

As an incentive to completing this short survey, we are offering you the chance of winning two tickets to this seasons FA Cup Final at Wembley in May 2022 (U18's must be accompanied by an adult). 

To be in with a chance of winning, we will need an email address for parents/guardians of entrants under the age of 18. For safeguarding compliance, we have to be sure that parents are aware of the involvement of their children. We will complete the draw following the deadline and contact the winners. The prize draw will take place W/C 03rd January 2022.

It is vitally important that anyone aged under 18 must have parent/guardian permission to complete this survey. The closing date is 3rd January 2022 (9am).
