When you have received it, please ensure the information we have is correct. There are spaces provided for any changes. We will send a separate form(s) if you have a Women’s or Youth section.
If you have not received a form by 1st May 2008, please contact us or phone 01726 74080. Please also phone 01726 74080 or contact us if you do have any queries about the new system.
Your club’s affiliation number will be forwarded to you when we have checked your affiliation and remittance forms are correct.
ALL CLUBS: Return forms and remittances arriving after 31st May 2008 will attract a £25 fine.
