Public Liability and Personal Accident Scheme 2008-09
Our agents are Budden Smale Westinsure. The Personal Accident Insurance will continue to be provided by Allianz plc via SBJ Sports.
After careful consideration it has been decided to change the Public Liability providers from CNA Insurance to Sportscover, again in conjunction with SBJ Sports, under the Countycover contract. This policy automatically includes
• Public & Products Liability
• Employers Liability
• Legal Defence costs in the Event of a ‘Player to Player’ incident
• Officers & Committee Members Liability
• Professional Indemnity
• Fundraising events up to 5000 attendees (excluding hazardous events such as fireworks, bouncy castles etc)
The Application Pack for the 2008/2009 Public Liability and Personal Accident Scheme is available on the right. Please complete and sign the SBJ Sports Football Solutions Application Form and Cornwall FA Remittance Form and return these together with your payment to Buddens Westinsure, 1 Sandpiper Court, Harrington Lane, Exeter, EX4 8NS.
Cover will be effective from 1st July 2008, or on the date payment is received whichever is the later, and will expire on 30th June 2009.

Claims Procedure For Personal Accident to Players
The following details should be notified to BuddenSmale Westinsure as soon as possible after an injury has occurred, which might give rise to a claim:
Name of Player
Name of Club
Nature & date of injury
You will then be provided with a claim form which should be completed in full (including doctor's statement) and returned to BuddenSmale Westinsure together with:
Copy of Medical certificate
Evdience of employment being a copy pay slip or copy of Accounts if the player is self-employed.
The Insurers, ACE Europe, make benefits payments in arrears and normally made at the end of the period of disability. If, however, the player would like a payment prior to the expiry date of a long term medical certificate, the player will be provided with a letter which should be signed and dated and returned to BuddenSmale Westinsure. A payment will then be issued up to the date of signing the letter.
